Retailers who deal in cigarettes, electronic vaping devices, and tobacco products are required by Wisconsin Statute and Village Ordinance to obtain a local license. To apply, the following forms are required:
CTV-100 - required annually for licensing
CTV-101 - required annually for EVERY member of the business
CTV-102 - required with the initial license application, or any time there is a change in Agent
